Question from Ananya, 29 — Bangalore

"I've had PCOS for 4 years. My doctor just says lose weight and put me on metformin. My periods are still irregular. What am I missing?"

Dr. Raga's Response

Ananya, metformin addresses insulin resistance — which is just one of PCOS's five root causes. If your PCOS is driven by gut dysbiosis or HPA axis disruption (stress hormones), metformin alone will not regulate your cycle. I'd want to know your LH:FSH ratio, fasting insulin, SHBG, and gut health markers before recommending anything specific. Losing weight is an outcome, not a cause — your body needs the right metabolic environment first.

Question from Preethi, 34 — London

"I moved to the UK 2 years ago and my weight has gone up 8kg despite eating the same. My thyroid tests are 'normal' according to my GP. What's happening?"

Dr. Raga's Response

Preethi, UK winter means your Vitamin D has likely crashed — and Vitamin D is not just a vitamin, it is a hormone that regulates insulin sensitivity and fat storage. UK labs use Western reference ranges; your TSH might be "normal" by UK standards but suboptimal for an Indian woman's physiology. I also want to know if your gut microbiome shifted with the diet change — that alone can cause 6–10kg of inflammatory weight. This is absolutely fixable.
